Wellness Package Guidelines
Additional fees apply for any illness related diagnostics, emergency examinations that require staff to stay after business hours and all products. Additional fees may apply for additional anesthesia and/or mature dog spays or neuters.
In the event that the owner fails to return for immunizations as recommended, the owner will be responsible for fees associated with restarting the immunization series.
Credit or debit card on file required for enrollment. Payment will be processed and billed to credit card on file on first business day of each month following enrollment for those clients electing the monthly payment option.
Benefits will be suspended in the event the credit card is declined or cancelled. Owner will have 10 days to submit new credit card information, otherwise enrollment will be cancelled, payment will be due in full immediately and will be subject to collection activity.
Wellness packages are non-refundable and non-transferable to other patients or clients.
For adult or senior wellness packages with dentistry, the dentistry can be performed after the 7th monthly payment has been processed or at any time with prepaid packages. In the event that there is greater than Grade One Dental Disease, the owner will be responsible for the additional fees. ULVH will provide a written treatment plan and associated costs prior to performing the dentistry.
For puppy wellness packages with spay; the surgery must be performed prior to the first heat, otherwise additional fees will apply.

Wellness packages are all inclusive. Credits or refunds will not be issued for services not needed or received within 12 -month enrollment period. Each pet’s vaccine recommendations are based on risk of exposure to that disease or illness. Credit will not be issued for vaccines not recommended or administered.
Enrollment period expires one year from date of initial down payment.
Owner is responsible for payments in the event of death, relinquishment or loss of pet once initial services have been rendered.
